Histopathologic findings. Hematoxylin and eosin (H&E)-stained sections demonstrate a densely cellular infiltrate with regions of fibrosis (A). The cellular areas are composed of lymphocytes, plasma cells, histiocytes, and eosinophils (B). Immunohistochemical stains demonstrate scattered CD3-positive T cells (C) and CD20-positive B cells (D), and abundant IgG-positive plasma cells (E), many of which are positive for IgG4 (F)
